How to sign up to NHS App:
To sign up for the NHS App please download the App to your mobile phone/device.
The benefit of the NHS App is you do not need to provide ID to the surgery as this is all done through your phone. If the system does not recognise your ID then you will be asked to come into the surgery to provide this.

Patient Access
How to sign up to Patient Access:
Register at The Surgery in person
Step 1: Read the information leaflet Online Registration Leaflet
Step 2: Download and complete our Online Registration Form
Step 3: Visit The Surgery in person and hand to reception your completed form.
Step 4: Provide reception with your identification (Passport / Driving licence etc.)
Register from home and visit The Surgery later
You can also register for this service online without visiting The Surgery by visiting the Patient Access Website. This method creates a basic account that allows you to book only one appointment. Using this option requires you to visit The Surgery after registering and bring with you your identification.
Step 1: Enter your postcode and click search
Step 2: Select your practice and click continue
Step 3: Click no to having a registration letter and then click continue
Step 4: Enter your personal details and click continue to complete the registration process
- Please note we look closely at all applications for access to make sure they are genuine
- Access can also be withdrawn at any time if there are concerns about patient confidentiality.

11-15 years
The Surgery does not offer online services to patients aged between 11-15yrs. NHS England recommend that practices adopt a very cautious approach to allowing parents to access their records. We have decided to adopt the most practical approach and withdraw parent’s access as soon as the child reaches 11 unless there are exceptional circumstances, each case will be assessed on an individual basis by the GP who will decide if the Proxy Access is authorised.
Abuse of the Online System
Abuse of the system includes, but is not exclusive to the following:
- Sending needless or abusive messages to the Practice
- Repeatedly booking and cancelling appointments
- Repeatedly booking appointments over a short period of time
- Booking appointments and not attending them
- Any offensive or abusive messages through the service
- Requesting prescriptions that you do not need.
Proxy access
If you are the parent of any children under the age of 12 you will need to fill in a ‘proxy’ access form to view their details. You can also grant access to a carer if you wish.
The Patient must give their consent before Proxy Access can be granted; this can be done by filling a form out from reception and for both the patient and Proxy to provide ID.
The Surgery may give formal Proxy Access to a representative of a patient who is not competent. The GP will look into this before any formal access is given.
Security
We take the security of your information very seriously. Our clinical systems provider ensures that the information you provide when using the online services portal is protected. All communications with the web site are encrypted, using the same high security that major banks use for their internet transactions (128 bit SSL). Only you can see the personal information you enter.
Using the most up-to-date version of your web browser also improves security of information. Remember to close the browser after use, especially if someone else could access your computer.
