Register with us as a New Patient

We welcome the registration of any new patients residing in our area. You will need to download and fill in the document below. You will then need to bring these into the Surgery with two forms of ID. For more information, please contact the Surgery.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Please note if you are registering babies or children at the Surgery, a parent or guardian must also be registered at the Surgery.

You will be registered on the list of a particular GP, however, you are welcome to seek medical advice from any GP or nurse of your choice. In the interest of consistency and continuity of care we would ask that, where possible, you consult with the same person for on-going issues.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.
